How did it go for you? I got my 3rd dose yesterday and holy shit it was painful. I remarked that it was about one-third of a bout with sepsis (which I have the misfortune of experience to measure against) but all the same s/sx. Brutal. Still hurt 36h later but starting to feel better.

At least I have confirmation of a robust immune response?

My delt is sore AF, but with all three shots I have had no respiratory after effects.
My company all got ours early on and across all three different drugs, the response was completely random when looking at age, gender, etc. I got lucky.

I purposely chose the left arm because I made pizza for 16 people tonight (outdoor birthday shindig for our three kids- each was allowed to invite one friend and their family) and I wanted to be at my best.

Over here, we've had a 109% increase in new infections in the past fortnight so we are back to wearing masks in all public indoor spaces, and WFH is strongly advised.

As context, we have 74% of the entire population fully inoculated and 86% of 18 and older. (You can geek out on the numbers here if you like.)

Next door in Germany, infection rates are on a sharp rise too, and I believe their inoculation coverage is similarly high.

It's starting to look like boosters might be a necessity rather than an option...

Well the powers that be gave the nod for the mix-and-match so I opted for a heterologous prime boost. Followed up my J-brand (one and done, not anymore) first course with the M-brand second course. Some arm soreness, pretty mild for anyone who has raced a bike, and some brain fogginess and general malaise, not unlike a hangover. Feeing pretty OK now and much more confident in my defenses.

The vaccination status of 138 epidemiologically linked
index cases of 204 delta variant-exposed household
contacts was available (figure 1B, table 2). The SAR in
household contacts exposed to fully vaccinated index
cases was 25% (95% CI 15–35; 17 of 69), which is similar
to the SAR in household contacts exposed to unvaccinated
index cases (23% [15–31]; 23 of 100; table 2). The
53 PCR-positive contacts arose from household exposure
to 39 PCR-positive index cases. Of these index cases who
gave rise to secondary transmission, the proportion who
were fully vaccinated (15 [38%] of 39) was similar to the
proportion who were unvaccinated (16 [41%] of 39).

SAR is secondary attack rate, ie, persons infected by the index case.


Faster clearance with delta infections:

Although viral kinetics appear visually similar for all four
groups of cases, we found quantitative differences in
estimated viral growth rates and decline rates (tables 3, 4).
Population (group-level) estimates of mean viral load
decline rates based on ORF1ab cycle threshold value data
varied in the range of 0·69–0·95 log10 units per mL
per daxes 4; appendix p 10), indicating that a typical
10-day period was required for viral load to decline from
peak to undetectable. A faster decline was seen in the alpha
(pp=0·93), unvaccinated delta (pp=0·79), and fully
vaccinated delta (pp=0·99) groups than in the pre-alpha
group. The mean viral load decline rate of the fully
vaccinated delta group was also faster than those of the
alpha group (pp=0·84) and the unvaccinated delta group
(pp=0·85). The differences in decline rates translate into a
difference of about 3 days in the mean duration of the
decline phase between the pre-alpha and delta vaccinated
groups.


Viral load and age.

We estimated mean peak viral load for 50-year-old
adults to be 8·14 (95% CrI 7·95 to 8·32) log10 copies
per mL, but peak viral load did not differ by variant or
vaccination status. However, we estimated that peak viral
load increases with age (pp=0·96 that the slope of peak
viral load with log[age] was >0), with an estimated
slope of 0·24 (95% CrI –0·02 to 0·49) log10 copies per mL
per unit change in log(age). This estimate translates to a
difference of 0·39 (–0·03 to 0·79) in mean peak log10
copies per mL between those aged 10 years and 50 years.

Our findings help to explain how and why the
delta variant is being transmitted so effectively in
populations with high vaccine coverage. Although
current vaccines remain effective at preventing severe
disease and deaths from COVID-19, our findings suggest
that vaccination alone is not sufficient to prevent all
transmission of the delta variant in the household
setting, where exposure is close and prolonged.
Increasing population immunity via booster programmes
and vaccination of teenagers will help to increase the
currently limited effect of vaccination on transmission,
but our analysis suggests that direct protection of
individuals at risk of severe outcomes, via vaccination
and non-pharmacological interventions, will remain
central to containing the burden of disease caused by the
delta variant.

If reduced rates of transmission were the primary goal, different approaches would have been prioritized in vaccine design and development. The goal, which has been well accomplished, is to reduce the rate of positive cases which result in symptomatic severe disease.

I received a booster on Wednesday, flavor M, and was a bit tired Thursday but no significant soreness. My neighbor works (she’s an RN) for a county health department and she can bring about to expire doses home. She took care of us at the kitchen table- what service.

Both my wife and I are scheduled for our boosters next week. We're going with the same as our original 2 vax, Moderna. Neither one of us had any reactions to any of our pokes.

I'm also scheduled for a booster. The soonest I could get one with Walgreens locally was the 10th of November, at least without hunting around. When I went online to schedule it their website asked what my original brand was and it offered me booster appointments locally for the same brand, in my case Moderna.

I had assumed that this would be more or less a walk-in situation but that has not turned out to be the case.

I drove through a rainstorm and pulled into the RiteAid parking lot 10 minutes ahead of schedule…waited until 5 minutes before my appointment to check in as instructed and when I went to the door, the clerk greeted me and told me that their power had been out for 1/2 hour and that I needed to reschedule….bwaaaa bwaaaa. Soonest I can get back in line is next Thursday, so best out of three I guess.

A breakthrough case occurs in a person who is considered fully vaccinated.

To date, Oregon has recorded 35,424 breakthrough cases, with an average age of 48.

Of all breakthrough cases, OHA said 4.4% have required hospitalization and 1% ended in death.

Since the beginning of the pandemic, Oregon has had 363,648 cases of COVID-19 and 4,358 people have died from it.

perhaps it more useful to also stress the vaccine greatly reduces the absolute # of deaths and hospitalization cases. The rate of cases is NOW 3.5 times higher in the unvaccinated in OR. As the actual Oregon breakthrough report lays out, the vaccine is the best way to prevent spread and serious illness. Median age of breakthrough deaths was 80; with 21 of the 350 breakthrough deaths were under 60, and 178 of the 350 were over 80.

The separate OHA C-19 weekly report shows incidence of C-19 cases is 309 per 100k people in OR counties with vaccination rate <50%, and 132 per 100k people where counties have vaccination rate >60%. The delta (no pun intended) in vaccination status results is a significant and tragically unnecessary # of deaths and burden to the healthcare system.
